Highlights of the Memory Care Coordinator Position:

  • The Memory Care Coordinator oversees the overall implementation, delivery and coordination of the Connections Memory Care neighborhood resident services, including engagement and supports the MCD/HWD in non-clinical health services.
  • Coordinates engagement aspects of life for Memory Care residents including participating in the development of personalized plan or program for successful aging in community.
  • Ensures residents satisfaction and achievement of their realistic goals. Motivates residents to function at highest level of independence.

The base range represents the low and high end of the pay range for this position. Actual pay will vary and may be above or below the range based on various factors including but not limited to location, experience, and performance. The range listed is just one component of our total compensation package for employees. Other rewards may include annual bonuses, short- and long-term incentives, and program-specific awards.
